What Is a DA Form 3645?

DA Form 3645, Organizational Clothing and Individual Equipment Record is used to record issues and turn-ins of clothing and equipment to enlisted army members. DA Form 3645 1 or the Additional Organizational and Individual Equipment Record is used for tracking other items stocked by an issue point.

Using DA Form 3645 and DA Form 3645-1

Both Organizational Clothing and Individual Equipment Records should be prepared in three copies. The first copy is then kept by the issue point, the second copy is given to the soldier receiving equipment and the third is sent to the soldier's unit for filing and keeping.

What Is a Condition Code on DA Form 3645?

The item classification criteria on DA form 3645 is in accordance with Federal Condition Codes. Supply condition codes are always the first position of the two-character Federal code and are defined as follows:

  • Code A – New items;
  • Code B – Serviceable items that had been previously used;
  • Code F – Unserviceable items;
  • Code H – Scrap or salvaged items;
  • Code X is assigned if the item in question does not meet Code B standard and may not be repaired.

How to Prepare the Organizational Clothing and Individual Equipment Record

The process of preparing both the main and additional equipment record forms is as follows:

Step 1 - Determine the procedures used to account for OCIE (OCIE refers to any item of Organizational Clothing and Individual Equipment);

Step 2 - Prepare, print and hand out a DA Form 3645;

Step 3 - Prepare, print and hand out a DA Form 3645-1 for any additional items;

Step 4 - Correctly file both forms.

Step 5 - File document in the correct unit supply files.

Both Form DA 3645 revised October 1991 and Form DA 3645-1 revised December 1883 are currently valid and active.
