Authorization to Use Photographs and/or Audio-Visual

photographs and/or video that may pertain to me—including my name, image,

likeness, and/or voice without compensation. I understand that this material may be

used in CAEP’s publications, website, social media, recruitment or training materials,

or other related endeavors. This authorization relates to the event(s) noted below and

is continuous and may only be withdrawn by my specific rescission of this

authorization.
