The article “Preventing aspiration: A common and dangerous problem for patient’s with
cancer” was a beneficial resource for the development of this nursing care plan. It was a
systematic review meant to review and discuss evidence based nursing interventions to prevent
patient aspiration. The patient being assessed for this care plan has multiple risk factors for
aspiration, and had already developed one case of aspiration pneumonia. This made the Risk for
Aspiration nursing diagnosis a priority for his care. The Guy & Smith article begins by defining
aspiration and then discusses the common causes of aspiration in vulnerable populations. The
authors define aspiration as, “…the inhalation of oropharyngeal or gastric contents into the lower
airways, which can lead to pneumonitis or pneumonia” (Guy & Smith, 2009, p. 105). They list
some of the common causes of aspiration as drug or alcohol intoxication, diminished level of
consciousness, issues with the swallowing or gag refluxes, seizures, VAP, and tube feedings
(Guy & Smith, 2009). The article then delves into the signs and symptoms of aspiration
including gagging with oral intake, coughing, adventitious lung sounds, and increased secretions
(Guy & Smith, 2009).
After the authors discuss the common ways to diagnose aspiration they begin to assess
the possible appropriate nursing interventions to implement to prevent aspiration. The authors
discuss that the appropriate position for the patient is in semi-fowlers with the head of bed
elevated 30 to 45 degrees (Guy & Smith, 2009). The authors also address how patients on tube
feedings should be managed and how gastric contents should be aspirated before initiating a tube
feeding (Guy & Smith, 2009). Finally, the article discusses artificial airways and how they
