Chromebook Borrowers Responsibility Form

In order to borrow a Chromebook, please read and sign this form and submit it

at the circula on desk. You only have to sign this once. It will be kept on ﬁle.

 Patrons 8 years & older will be able to borrow a Chromebook for up to 1 hour at a me to use

within the library only.

 Children under 18 will be required to submit a form signed by a parent or guardian assuming full

ﬁnancial responsibility for the cost of repair or replacement, whichever is less, due to accidental

damage or misuse. Please ask at the desk in the Children’s Room.

 The Chromebooks will be circulated on a ﬁrst-come, ﬁrst-served basis. Holds cannot be placed on

the Chromebooks. Renewals are allowed based on availability.

 Chromebooks must be returned at the circula on desk where they were checked out.

 They must be returned 15 minutes before the library closes.

 Borrowers need to have a valid CLAMS library card in order to check out a Chromebook.

 Chromebooks that are not returned or damaged beyond repair will be charged the full

replacement cost of $350. If a Chromebook is damaged but repairable, the patron will be charged

the cost of the repair plus a $75 processing fee.

 A borrower agreement needs to be signed by ﬁrst- me users. Repeat users need not sign another

agreement. Circula on staﬀ will note on the patron's record that a borrower's agreement is on

ﬁle. By that agreement the borrower is subject to the same rules and requirements of the Library

and agrees to use it according to the rules of Library use. These include:

 Users must be courteous and respec ul when using computers and conform to the Library's Rules

of Conduct.

 Children under 8 years of age must be in the company of an adult at all mes while using a

computer.

 Parents are responsible for their child's Internet use.

 Library staﬀ may shorten a session if need demands it; or terminate a session if terminals are

used inappropriately.

 Chromebook use must conform to the library's internet policy.
