Follow this checklist and then submit all required documents and class schedules to the
Brookhaven College Veteran Services Office in the Student Services Center, Bldg. S, Room
S136 as soon as possible in order to complete the certification of your veteran educational
benefits in as timely a manner as possible. Thank you for your cooperation.
Student Certification Checklist
Complete and sign the Veteran Services Student Packet 

Provide Member 4 copy of DD214 

Submit transcripts to Brookhaven College (College and Military, Official
Transcripts only) 

Register for classes and submit the Registration Receipt

If Residency Code on
receipt =3-Out-of- State see below for further instructions) 
 * The following steps must be
completed to receive Veteran Eligible In-State Tuition. Waiver must be submitted each
semester until TX residency is established by the college.
1.
Completed & signed Out-of-State Residency Waiver Request 

2.
Copy of DD214 

3.
Submit waiver and copy of registration receipt to Hazel Beale in Registrar’s Office
(Window #1 in Admissions) 

Provide the necessary Veteran Educational Benefit proof (see
individual chapter/benefit requirements below):
1.
[CH30 & CH33 ONLY] Certificate of Eligibility (COE) 

2.
[CH1606/1607 ONLY] Provide Notice of Basic Eligibility (NOBE: DD
Form2384) 

3.
[CH31 – Voc Rehab ONLY] Complete paperwork with your Vocational
Rehabilitation Caseworker (VA 22- 
 1905) 

4.
[CH35 – Dep/Spouse ONLY] Provide Certificate of Eligibility (COE) 

5.
[HAZLEWOOD] Hazlewood Application & Copy of Hazlewood Student Hours
for that semester

PLEASE NOTE: 
 During peak periods (beginning of semester) processing times can take 30-
45 business days from the date submitted to the Veteran Services Office. Student requests
are processed in the order they are received. 

CH33 Post 9/11, CH 31-Voc Rehab, and Hazlewood ONLY: Upon receipt of ALL required
documents for certification, our office will place a hold on your courses to prevent you from
being dropped for nonpayment. 
 Students using any GI Bill should be aware that the VA
will only pay for classes that are REQUIRED for the completion of your selected degree/plan
of study. Please follow your degree plan to avoid non-payment and/or non-certification of
classes.
