The undersigned recognizes the right of Altitude Gym to proceed without any

compensation, the uses and publications of my picture or image in the documents

explained in the preceding paragraph. The undersigned shall indemnify Altitude

Gym of any claim arising from the use and/or the publication of my visual image

by anyone, including third parties.
