VERIFICATION OF VETERAN STATUS
You reported that you are a Veteran of the U.S. Armed Forces on your Free Application for Federal Student Aid
(FAFSA). However, the Veteran’s Administration was unable to confirm your status as a Veteran honorably discharged
from the U.S. Armed Forces. In order to proceed with your application for financial aid, you must provide our office
with documentation to confirm your Veteran status. Please reviews the information requested below and attach the re-
quired documentation to this form.
I certify that I am a Veteran of the U.S. Armed Forces. As proof of my military status, I have attached a copy of the DD-214
form (military separation form) which was issued to me upon my discharge from the military. Note: You should also contact
a local regional office of the Veteran’s Administration to ensure that your records are updated accurately. Make sure that the
copy of the DD-214 includes your discharge status. Your Financial Aid eligibility is based on a discharge other than dishon-
orable.

If your Dependency status was based solely on you being a veteran, and if you are not a Veteran of
the U.S. Armed Forces, corrections to your Student Aid Report are required. You are required to
include parent (s) income information and obtain their signature on your Student Aid Report. Your
application requires resubmission to the USDE Central Processor to include this additional infor-
mation. Contact the Financial Aid Office for assistance in completing these corrections.
