When a State of Registry or State of the Operator has cause to de-register of an aircraft or withdraw the
approval of an operator/aircraft for operations within the RVSM airspace, details as requested below must be
submitted to the Australian Airspace Monitoring Agency (AAMA) by the most appropriate method. This form should
also be used to notify the AAMA of a de-registration from a state's registry.
