This form is a non-disciplinary form and is to be used to address issues concerning non-culpable
absences.

This meeting is occurring because the records of Health PEI demonstrate that the employee’s
non-culpable absences have reached or exceed one hundred twelve and a half (112.5) hours
within a 12-month period.
It is confirmed that, at this meeting, the following occurred:
1.
The employee has been advised that his/her non-culpable absenteeism rate may
place his/her job in jeopardy and that his/her failure to improve non-culpable
absenteeism immediately could result in the termination of his/her employment.
The employee was specifically advised that, in accordance with Step 4 of the
Attendance Management Policy, if the employee’s non-culpable absenteeism
reaches or exceeds one hundred thirty-five (135) hours within any 12-month
period and the employee cannot provide significant assurance to Health PEI that
there will be regular attendance in the future, then Health PEI may terminate the
employment of the employee for non-disciplinary reasons.
2.
The employee was provided with the opportunity to explain what he/she will be
doing in the future to ensure he/she will be able to attend work on a regular basis
and in a manner that is in full compliance with the Attendance Management
Policy.
3.
The employee was advised of the availability of assistance through EAP and,
further, that if the employee wished to have any assistance, he/she could contact
Human Resources immediately.
A copy of this summary will be kept in the employee’s personnel file, with a copy

provided to the employee and to the Union.
