On-Campus Proof of Residency Letter Request Form

For International Students in F-1 or J-1 Status

Please complete this form to request a Proof of Residency Letter. The letter will be issued

within two business days from the date of your request and you will be notified when it

is ready. You may pick up the letter at the Slater International Center during regular

business hours (8:30 am – 4:30 pm).

If you have any questions regarding the procedure for obtaining a Massachusetts’s

driver’s license or denial letter, please refer to the International Student Handbook

section on “Driving in the United States” or speak with your International Student

Advisor.
