How to Enroll in TEENS
(Teen E lectronic Event Notification S ervice)
The TEENS Program is a voluntary FREE service provided by the New York State Department of
Motor Vehicles that informs the parent of a minor (under the age of 18) when a conviction,
suspension, revocation, accident, or ticket is posted to the minor’s license record.
Preconditions for Enrollment
The following conditions must be met to enroll in the program:
•
The child is under the age of 18
•
The parent/guardian has a NYS license or non-driver ID
How to Enroll
There are three ways to enroll in this program:
1. The minor’s parent/guardian can indicate that they would like to participate in TEENS by providing the
parent/guardian’s Client ID # (CID) in the TEENS section of the ‘Parent/Guardian Consent’ box on the
minor’s Application for Driver License or Non-Driver ID ( form MV-44).
2. Parents/guardians may sign up to receive electronic notifications through participation in MyDMV. First, go to
https://my.dmv.ny.gov and complete your MyDMV Enrollment. Parents/guardians will then be able to enroll on-
line by selecting the TEENS application and providing the necessary information regarding their minor.
3. The parent/guardian can obtain form MV-TEENS (Teen Electronic Event Notification Service) Enrollment
and Consent. This form can be obtained two ways:

Ask for form MV-TEENS at a NYS DMV issuing office.
The parent/guardian must fill out the form completely. The following information is required to fill out the form:
•
Parent/guardian’s name
•
Parent/guardian’s Client ID (License or non-driver ID #)
•
Minor’s name
•
Minor’s Client ID (License or Non-Driver ID #)
•
Minor’s birth date
•
Parent/Guardian’s relationship to the minor
•
Parent/Guardian’s signature
Submit the Form
After completing the form, submit it by mailing to:
