Amplify Credit Union will beging processing the request once all required items are received. Please

fill out and print form. Processing time is approximately 10 business days from date all required

items are received.

Completed Subordination Request Form

Subordination Agreement – Prepared by attorney or title company

Amplify Credit Union can prepare the agreement for an additional document

preparation fee of $140.00 in addition to subordination processing fee.

Fee must be submitted with request.

1003 – Uniform Residential Loan Application

1008 – Uniform Underwriting Approval or Transmittal Summary

Title Commitment [Must be dated within 60 days of request]

Property Appraisal

[Appraisal must have been completed within 90 days of this request. If an appraisal is not

being used for new loan, please provide signed letter of explanation detailing reason.]

Borrower’s signed authorization

$150.00 Processing fee. Please make check or money order payable to: Amplify Credit Union

Fed Ex or UPS Shipping Label
