Photo/Name/Work Release Form

From time to time, Global Montessori School (GMS) students are photographed during

academic, artistic or athletic endeavors. Students are also routinely photographed for

inclusion in school related promotional and advertising materials that are disseminated to the

public through newspapers and other forms of advertising, admissions materials, our web

site and other media. The release below allows us to use your child(ren)’s photo/name/work

in such endeavors.

Please check each box that applies. One release form may be used per family.

I give permission for my child(ren)’s photograph to be used by GMS (photo only, no

names will be used).

I give permission for my child(ren)’s name to be used by GMS (first name only).

I give permission for my child(ren)’s schoolwork to be used by GMS (work will be

posted with first name only).

I do not give permission for my child(ren)’s photo, name or work to be used by

GMS.
