to obtain your Return Authorization number. Our ofﬁce is open from 9:00-4:00 Monday through Friday, excluding

all holidays.

We will email you a pre-paid return label to the address we have on ﬁle unless instructed otherwise.

Please print out this form, complete the necessary ﬁelds and send it, along with your mostly unused haircare in its

original packaging, to:

As soon as we receive the returned items, we will process your refund for your original purchase price (minus any

discounts) and will refund all qualifying returns to your credit card, PayPal or Amazon account within 7 days of
