18. Permanent Address and Phone (usually Home of Record). This is the address
where you live on a more permanent basis, where you receive mail, and the location
from which you will leave for training and returning to after training.
19. Current Mailing Address and Phone. If you have are living in a temporary
location (less than 6 months) and will be moving back to your permanent address
before your appointment is complete, enter your current address here. If you
have only one address, leave this blank.
20. US Citizen. Mark YES or NO.
20a-c. Native. MUST mark YES or NO.
If you were born in the US or in a US Territory and/or have a US Birth Certificate, then
you will mark YES. If you are not a native citizen, you will mark NO, then mark the
appropriate block in letter b, and enter your Certificate Number, along with the date,
place, and court from which it was issued.
21a-e. Complete all sections: Note: High school name and address city, state and zip.
(The form has an error and will not automatically move to this block. You will need to
select the block manually by If the form will not allow you to complete this field, print and
handwrite the school name, city, state, and zip.)
22a-d. Highest level service school attended (Military School only). If none, leave
blank.
23a-b. Enter any foreign languages and level of proficiency. Example: Spanish -
Fluent in writing and speaking. (b) If you know your ALAT Score, enter it here.
24. Conscientious Objector: Enter YES or NO. If YES, waiver is required. Not eligible
for appointment if YES.
25. Assignment/ Dependent Care Family care plan for single with dependents
required. Box must be marked/checked.
26. Must Mark Yes or No. If you mark Yes and you did not request a waiver for the
offense/incident when you submitted your online application, you will need to provide
a written waiver request.
27a-g. Active Military Service (Current or Prior military service only). Dates must be
sequential, with any breaks indicated. For Reserve Components, if mobilized or
deployed, enter period here. Date format: DD MMM YYYY
