As a member of a Palo Alto High School athletics team, I request a WAIVER/PREP during my

Physical Education class.

I understand I must attend Physical Education class until my teacher receives this form completed and

signed by me, and my parent, and excuses me from Physical Education.

When any of the following occurs, it is my responsibility to return to my Physical Education class the

next meeting. Failure to do so may result in loss of credit and/or lowering of your grade including

failing and receiving no credit.

1. I quit the team.

2. The coach dismisses me/cuts me from the team.

3. I completed my last athletic contest of the season. Your coach may NOT excuse you for any

practices after this date.

4. During fitness testing week (May, even while competing in CCS)
