FISH MOVEMENT REPORT FORM AND CRITERIA
This paragraph is to inform fish operators who import fish into Utah by vehicle that they
are required to stop at Utah Ports of Entry upon entering Utah and show their Utah
Livestock and Fish Movement Report
(UL&FMR) form. Persons who import fish into
Utah by airplane are also required to present their UL&FMR form at the customs office
at the airport. Before any fish can enter the state a Utah Livestock and Fish Movement
Report (obtained prior to transport fro m Fish Health Inspectors) shall be presented at the
port of entry or the airport by the importer. All transport vehicles carrying aquatic
animals imported into Utah or transported through Utah must posses proper
documentation and approval (R58 -17-13(I)). Port of entry and customs officers forward
the Livestock and Fish Movement Report to the Utah Department of Agriculture and
Food for review by Fish Health Inspectors. It is our intent to prevent the entry of any
diseased aquatic animals into the State of Ut
ah. Therefore, we appreciate your
cooperation in this regard. Enclosed is a copy of the Utah Livestock and Fish Movement
