This form is to be contained in the Ground Vehicle Book until used In Case of Accident

TAKE CARE OF INJURED

NOTIFY APPROPRIATE POLICY DEPARTMENT/AGENCY

MAKE NO STATEMENTS EXCEPT TO POLICE

SIGN NOTHING

RECORD THE FOLLOWING NOTES:

Date, Time, Weather

Name, Address and Telephone of other driver and passengers

Name, Address and Telephone of Witnesses

Name, Address and Telephone of Police

Other Driver's License State and Number

Other Vehicle License Number, State, Insurance

Company, Insurance Policy Number, Vehicle Make,

Model and Year (VIN if possible)

Notify the Squadron Commander (Wing Commander if NEWG is custodian)

Notify the Unit Safety Offcier (Wing Safety officer if NEWG is custodian)

The squadron/wing commander will notify the additional contacts as indicated in NEWG Supplement 2

NOTE: Fill in the Nebraska Accident Report (DR Form 41) as soon as possible.
