Comment/Suggestion Form

The CGSA Comment/Suggestion Program is designed to provide every player/parent an equal opportunity to share his or

her ideas, concerns or suggestions on how we can make CGSA function better. Although the form asks for personal

information, anonymous suggestions may also be submitted and will be considered. Of course, providing contact

information allows for follow up questions and discussion regarding the suggestion that have been submitted. The CGSA

Suggestion Form does not deal with grievance matters, i.e. problems on the team or personal grievances as those items

should be submitted through the normal channels in place to deal with such matters, starting with the division

commissioner (as listed on the CGSA website).
