End of Year Evaluation

Form A is assessed and updated by the Mid-Year Progress Report and as part of the Year-End Evaluation

•

Form A IDP Performance Goals are to come from the most recent Year-End Evaluation

•

Form A may also be used for a teacher rated Effective or Highly Effective on their most recent Year-End

Evaluation if the evaluator decides a written improvement plan is warranted
