We the Elders at COTR are grateful to God for the opportunity to train up men for future ministry

through the pastoral internship program. We are grateful as well for your desire to become an intern

at COTR. In order to help us as Elders minister to you more effectively and evaluate whether you are

ready for this internship, please honestly answer the questions below. Also, if you are married, there

are questions for your wife’s response. Know that the answers are confidential and will only be seen

by the Elders of COTR for the purpose of your spiritual growth and in assessing whether the pastoral

internship at COTR is right for you at this time. Thank you for taking the time to fill out this personal

evaluation form. We pray it will be for your growth in Christ-likeness, and in that the glory of God.

an additional page. Please fill out your responses in the spirit of Matthew 5.16, “In the same way, let
