1Subcontractor’s Unconditional Final Affidavit & Release of Lien
The undersigned has been paid and has received final payment in the sum of $____________________ (current
billing amount) for labor, services, equipment or material furnished to CYE Enterprises Inc. on the job of
____________________________________ located at _______________________________ and does hereby
release any mechanic's lien, stop notice or bond right that the undersigned has on the above referenced job to the
following extent.

undersigned,
upon
payment
of
this
draw
and
in
consideration
of
the
total
sum
of
$_______________________ (total contract amount including change orders), does hereby waive, release,
remise and relinquish the undersigned’s right to claim, demand, impress or impose a lien or liens for all labor and
materials in place at the time of the execution of this affidavit.
This undersigned further certifies that all work required under this contract has been performed in accordance
with the terms thereof, and that there are no known mechanics or material liens outstanding as of this date. That
all subcontractors, material men and laborers who have furnished materials, labor or services to the above
described property have been paid in full for the work, material and services furnished and performed as of this
date.
