CHILDREN’S MEDICAL INFORMATION
If your child needs medical, dental, hospital or other health
services, you as parent must give permission. It’s the law.
What about the times when you cannot be reached for
permission? That could mean care would be delayed until
consent is obtained. Except in a life- or limb-threatening sit-
uation, emergency care may be rendered to a child only
with the consent of the parent or legal guardian. Sometimes
a child may need care which is not a true emergency. In

sion means unnecessary anxious moments for the child.
You can prepare for such unexpected situations when you
are away. Make sure babysitters know how to reach you at
all times. And when you know you will be hard to reach,
you can give permission to other adults. They can then act
for you.

responsible for medical care for your children when you are
not available. Fill out this form carefully. Have your signa-
ture witnessed. Put it in a place that is easy to find. Tell
babysitters and others where to find it.
