PROCEDURE FOR RETURNING BMV PRODUCTS

1. Only one location per form. Please DO NOT list products from more than one location on

this form.

2. Please return this completed form and all products to:

Bureau of Motor Vehicles

Return Tag Unit

Harrisburg, PA 17104

3. Each product being returned should be listed on this form using the assigned .centric product

class and type.

4. A description of the product, for example, the month and year of sticker, the quantity of

products and the beginning and ending numbers of the products listed in sequential order,

must be included on this form.

5. All expired registration stickers must be returned within 30 days after their expiration.

6. Registration stickers and other products assigned to you from the On-Line Business Partner,

are to be returned to the On-Line Business Partner from which you received them. The On-

Line Business Partner will be responsible for returning the product to PennDOT.

7. This form MUST be used to return registration stickers to PennDOT.
