1) Family Life First (FLF) accepts the services of the said volunteer and is committed to:

 provide supervisory aid and reciprocal feedback on the volunteer’s roles of duties

 respect the said volunteer as our valued partner in fulfilling FLF’s client-centred objectives.

2) I, ______________________________ (Volunteer’s name) NRIC No. ___________________

hereby agree volunteer my service wholeheartedly and accept:

 that I shall abide by and follow the instructions/directions provided by FLF, or its framework,

policies, rules and procedures.

 the code of confidentiality with regards to any information related to the corporation /

customers / volunteers.

 that I must be punctual for my volunteering service

 that I should inform the FLF if I am medically unfit to carry out my duties;

 that during my volunteer service, I shall not coerce / entice any of my charges into any

agreement or transaction that would benefit my personal financial interest (direct or indirect).

Examples of these include BUT are not limited to direct / indirect selling of consumables,

investment-related products: fund investment, insurance, real estate, etc;

 that I shall not offer any service including but not limited to tuition services, home nursing

services, etc. or my personal details to my charges, their caregivers or other volunteers in our

programmes without seeking prior approval from FLF

 that I shall be sensitive to and always respect the privacy and cultural/religious background of

our clients.

 I shall give one week of notice to FLF should I wish to resign from volunteer service;

 FLF may terminate this Agreement without cause upon giving thirty (30) days’ prior written

notice to each other
