Notice of Transfer
A student who changes schools after initially attending another school is subject to Bylaw 9.3 Transfer Regulations. The
student may be ineligible for the remainder of the sport season during which the transfer occurs. However, Bylaw 9.3.2.1
allows the student to transfer to another school prior to the first day of practice in a sport in which the student wishes to
participate and maintain his/her eligibility, provided the public school board or private school policies allow the student to
transfer and maintain eligibility (reference FS 1006.20). By submitting this form to FHSAA, the member school principal
declares this student is eligible under the provisions of B.9.3.2.1. The information included in this form may be shared
with the previous school.
The student must meet all other eligibility requirements to maintain eligibility. The school/student must complete a
GA4 Affidavit of Compliance with Policy on Recruiting, where applicable.
A student who is ineligible at the time of his/her transfer may not regain eligibility through the use of this form.

1. This form must be initiated by the Principal of the school receiving the student, signed and notarized.

form.
4. This form must be completed and submitted for each school the student attended during the current school year, prior to the
student’s participation at the receiving school.
