Note :

Each applicant must fill in 3 forms.

Each applicant must forward three recent Identity Card size photographs.

Each applicant must be made on proper forms:

to apply for Diplomatic Identity Cards, applications have to be made on Form D 4A (white);

ii.

to apply for home-based Non-Diplomatic Identity Cards, applications have to be made on Form D 4B (blue).

Application forms must be signed personally by the Applicant.

At the expiry of tour of duty in Brunei Darussalam, the holder must surrender the Identity Cards to the issuing

authority.

The Identity Card is valid for three years, reneral on expiry.

Loss of the Identity Card must immediately be reported to any Police Station. A copy of such report must be

forwarded to the issuing authority.

Children above the age of 12 years will be required to apply for Identity Cards.
