When communicating with parent(s)/guardian regarding arranging a meeting, you must include
the following information:
Purpose
Date
Time
Location
Who will be in attendance.
Provision relating to other individuals who have knowledge or expertise about
the student may participate in the meeting at the discretion of the parent or the
school.
If student is age 14 or in grade 8, indicate that a purpose of the meeting will be
to consider a transition plan.
Invite the student if transition planning is part of IEP.
For students age 16 or older, indicate purpose of meeting is to consider
transition services.
Identify any other agency that will be invited to send a representative to the
meeting.
