The fee schedule applies to the site of treatment, e.g., if the treatment was provided in

Cook County, follow the maximums of Region 1.

Hospital fee schedules:

Cook, DuPage, Will, Kane, McHenry, DeKalb, Kendall, and Grundy Counties;

Kankakee County;

Madison, St. Clair, Macoupin, Clinton, Monroe, Jersey, Bond, and Calhoun Counties;

Winnebago and Boone Counties;

Peoria, Tazewell, Woodford, Marshall, and Stark Counties;

Champaign, Piatt, and Ford Counties;

Rock Island, Henry, and Mercer Counties;

Sangamon and Menard Counties;

McLean County;

10. Lake County;

11. Macon County;

12. Vermilion County;

13. Alexander County; and

14. All other counties of the State.

Non-hospital fee schedules:

Cook County;

DuPage, Kane, Lake, and Will Counties;

Bond, Calhoun, Clinton, Jersey, Macoupin, Madison, Monroe, Montgomery,

Randolph, St. Clair, and Washington Counties; and

All other counties of Illinois.
