2015 ANNUAL MEETING VOTER FORM AND PROXY VOTER FORM

The Oak Lakes Estates HOA Board would like to begin by offering our sincere thank you to each and every one of you for

your support over this past year. This form is intended to be a legal instrument for each HOA member household in good

standing to use for casting their votes, or to allow a proxy to cast their votes in their stead. If you cannot vote in person at

the Annual Meeting on Saturday, June 6, 2015, simply print, vote, and mail this in; or have an HOA member‐neighbor IN

GOOD STANDING bring or mail it for you, or fill out the proxy info with an HOA Member‐neighbor IN GOOD STANDING’s

info, sign, and thereby allow your neighbor to vote for you. Please remember that if you sign and hand to a fellow HOA

Member to vote as your proxy, and this person is in good standing with the HOA (meaning all dues are paid, no

outstanding liens or notices, etc.) then whatever they decide will count as YOUR VOTE!

HOA Board of Directors’ Proposals for 2015:

1. Should the HOA allow online voting, in addition to write‐in and in‐person voting?

2. Should the current Declaration and various Section CC&Rs be replaced with one set of

rules for everyone, eliminating the section designations, and uniting the entire Oak Lake

community under one clean, clear set of standard HOA documents?

3. Should the HOA allow everyone who uses one or more of the 4 Oak Lake entrances to opt‐

in their property permanently, thus increasing the funding for the HOA and allowing

these members (in good standing) to also legally use the park and lake access areas?

4. Putting up a gate and fence at this park is crucial to protecting the common area for future

development and safety. The Board has come up with two proposals, below.

a. One‐time Special Assessment of $50 per household to pay for gate and fence. ______Yes

b. One‐time Special Assessment of $50 per household, refunded by reducing dues

by $25/yr over the following 2 years for all who paid the Assessment.

We must have a quorum to have a legal vote. Please either attend the Annual Meeting, or fill out & mail this form to the

HOA Board, or give to your designated proxy to bring to the meeting or mail to the HOA Board.

The HOA Board must receive all votes no later than July 7, 2015. Any votes received after that date will not be counted,

regardless of Quorum issues.
