EXTENDED DAY OVERTIME FORM
This form is to be completed by each Extended Day Aide, Assistant, and Supervisor who performs approved overtime.
Overtime is defined as any time worked outside of the employees regularly scheduled shift time.
This form must be submitted to the Extended Day Central Office by no later than the deadline to enter into Oracle for
the pay period the time is worked.
