Are there any salary changes this month? Have you provided us with
new annual or hourly rates?
Have you provided overtime figures?
Have you provided commission figures?
Have expenses details been provided?
Are there any new Attachment of Earnings to be processed?
Have you provided us with changes to Pension contributions?
Has anybody joined / left the Pension scheme?
Are there any change of names and / or addresses?
Are there any new new employees to be set up?
o If so have you provided us with a completed ‘Employee Details Form’?
Are there any leavers to process?
o If so, what are their leave dates and is there any outstanding / overtaken
holiday to be calculated?
Has anybody taken any time off sick?
o Is this to be paid or unpaid?
o Have they provided you with a self certificate / doctors certificate?
Has anybody started Maternity leave?
o Have you provided us with their MATB1 form?
Has anybody been off on Paternity leave?
o Have you provided us with their SC4 form?
