Take your time and please evaluate your players honestly and to the best of your knowledge.

This form will be used next season, to evaluate the players and to help form teams as evenly as

possible. By completing this form, players will not have to attend assessments for next season.

Please evaluate players with a number 1, 2 or 3 in each category. Number 1 = beginner player, 2

= average and 3 = very good. A 2+ may be used for above average, if needed.

Hitting

Good form, makes contact, ability to base hit consistently

Pitching

Good form and is able to pitch strikes consistently

Catcher

Has the ability to play the catcher position and stops the ball

Fielding

Has ability to field grounders and catch fly balls consistently

Throwing

Good form, throws hard and can throw at target consistently

Player is aggressive and shows enthusiasm towards sport

Attends practice & games consistently

DEADLINE: FRIDAY, JULY 13 (prior to 5:00 p.m.)

In order to receive a $20 credit on your account; the Coach Feedback Form,

Player Evaluation Form, Player Emergency Information Forms and the team equipment

must be returned to the Woodland Park Parks and Recreation Office before the listed

deadline. Team coaches need to coordinate this effort together, so everything is

returned.
