To be eligible for the issue of a Low Volume VIN (LVV) you must complete this form and provide evidence of your identity (EOI). Refer

to the separate Evidence of Identity Information Sheet (F4362) or visit for requirements.

The Department of Transport and Main Roads will issue a maximum of three LVVs in a calendar year. Manufacturers of trailers

requiring more than three LVVs in a calendar year must apply direct to NEVDIS via email at .au to create their own
