Cooperating Teacher Form Submission Checklist

During the final week of the internship semester, Cooperating Teachers are asked to complete

evaluations of the internship experience. The feedback provided on these forms is utilized by the College

of Education to enhance the learning experience of the students. Please be assured that all feedback

offered remains confidential.

document form submission. Student interns are required to bring this document to the final seminar as
