HOUSING NOTICE
FOR PERSONS WITH DISABILITIES
The DuPage Housing Authority (DHA) is opening the waiting list for persons interested in living in Supported
Housing designed to meet the needs of persons with disabilities. Supported Housing is an apartment unit that
has on-site services available to all residents. The ideal candidate for these program will have a diagnosed
disability that requires one or more of the following support services:
1. Mental Health Counseling, Individual or Group Therapy
2. Medication Monitoring and assistance with Self-Management
3. Case Management Services
4. Employment Services
5. Assistance with money management and / or budgeting
6. Support and monitoring of personal care, housekeeping, and health and nutrition services.
All Support Services are provided by Trinity Services, Inc.
On the Waiting List Application individuals may indicate their interest in one or more of the following Supported
Housing settings.

Lombard (Opening late 2015) 
 
To indicate your interest in this housing you must:
1. Complete the DuPage Housing Authority Supported Housing Projects Waiting List Application.
2. Indicate the project for which you would like to be considered (Landings on Villa or Findley
Supported Housing).
You may indicate that you want to be considered for the current
development or for the setting that will open in Lombard sometime in late 2015 or for either
development.
3. Indicate the size of the unit you require. Persons seeking a two bedroom unit must meet the HUD
requirements for a two bed-room unit (based on size of family, services of a live-in aide, etc.)
Applicants will be placed on the Wait List based by date order their application arrives (as provided on the post
mark or date stamped by DHA when the application is returned in person. Placement on the Wait List does not
guarantee either eligibility for the program or an offer of housing. The order in which applicants receive
services is impacted by the preferences for which they qualify.
Preferences:
Residency Preference: Applicant is a resident of the DHA service area or employed in the DHA
service area. Residence and employment and must be on a permanent, non-temporary basis.
Family Preference: At least one adult who will be residing in the household is 62 years of age or
older or the household will have at least one disabled family member.
Working Preference: At least one adult in the household is employed at least 30 hours per
week or is a participant in an accredited employment training program or the head, spouse or sole
member of the household is 62 or older or is a person with disability.
Veterans, Victims of Domestic Violence, Family Self-Sufficiency Program Participants or an
applicant who is in a HUD approved Demonstration Program.
Involuntary Displacement Preference: If Government action or a natural disaster has
involuntarily displaced the applicant.
Homeless: Applicant is transitioning from a homeless services program (such as transitional
housing or permanent supportive housing) or is residing in a shelter or other location not fit for
human habitation.
DuPage Housing Authority will begin accepting applications for the Supported Housing Projects Waiting List on
Monday November 17, 2014 at 8:30 a.m. and the list will remain open until 4:30 p.m. on Wednesday,
November 19, 2014. Applications may be picked up that the DHA Administrative Offices located at 711 E.
Roosevelt Road, Wheaton, IL during regular business hours beginning October 15, 2014 or downloaded from
the agency website at .
Applications turned in after 4:30 p.m. on Wednesday, November 19, 2014 will not be accepted.
