Please review this form carefully as this is your final order. Sign and return the form with any additions or changes

LEASE READ AND SIGN

ASAP. Payment in full is required to complete the order. Your individual order is not complete until we have this signed form on file with

payment information. Once we receive all individual order forms, we will share a final contract with the bride. Once approved, signed and

returned, The Plumed Serpent Bridal (TPSB) will place this order with the designer. All sales are final and, as the dresses are made to order,

there are no returns or exchanges. All dresses are made to order, not custom made to measurements; therefore, alterations are usually

necessary. If you are between sizes on the size chart, always order the larger size. You can always have a dress taken in but it can’t always be

let out. The Plumed Serpent Bridal offers alterations on bridesmaids dresses for a fee of $150. Alterations at TPSB are offered on Mondays,

Wednesday & Thursdays and are by appointment only. If these times are not convenient to your schedule, TPSB can provide you with a list

of local and trusted seamstresses and tailors in the area who may better accommodate your schedule. We cannot be responsible for any

alterations made outside TPSB. Please allow at least 4-6 weeks for alterations. $25 charge for declined credit cards and bounced checks.

Any problems/issues with your dress (dress defects, size problems, etc.) should be reported to TPSB within 7 days of receiving your dress.

I understand and agree with all the terms stated in this Contract and Policy Form and authorize TPSB to charge the credit card on file.
