DOCTOR’S RELEASE LETTER
The attached release letter is a “last resort” item. It is designed to demonstrate how strongly you
feel about having your loved one discharged from hospital too early, and your concerns about
their ongoing need for care and treatment based on your past experiences and observations.
It is not intended to alienate the Doctor.
It is always helpful if we communicate in an appropriately assertive yet caring and respectful
manner. As a family member, you probably know your ill relative better than anyone, so it is
important that the doctor hear your concerns and respect the information you have to contribute.
Conclusions and decisions are only as good as the facts upon which they are based. It’s only
when family members are able to work together with professionals as a team that you can be
assured of the best treatment for your loved one.
If you can share these thoughts and your specific concerns verbally or by other means with the
doctor, it may eliminate the need for the attached letter.
It is always better if you can to take the least confrontational route to secure the best possible
treatment and care for your ill loved one. Focus on trying to work with the doctor, because he or
she should be your best ally in the delivery of treatment and care.
