Vessels used in commercial fi shing activities (commercial fi shing vessels, tenders, processors and transporters) must be licensed with the Entry Commission
by completing this form. If the vessel is also used for guided sport fi shing, please contact your local Alaska Department of Fish and Game, Sport Fish Division
offi ce to request the necessary forms. Fee categories are based on the overall length and are shown below. If the vessel is used for salmon seining, gillnetting
or trolling, complete number 22 and/or 23 in Part B. If you are not sure of the vessel’s ADF&G number, or whether it has one, please complete Part D on the
back of this page.

Fees are based on the overall length of the vessel, as defi ned by the United States Coast Guard. If this vessel is being licensed for
commercial fi shing in Alaska for the fi rst time, submit an additional $30 for the initial issuance of a metal ADF&G number plate.
