A child custody, physical placement or visitation order issued by the _________________ Court of the State of

judgment on ___________________, 20___ , together with all its accompanying documents and information.

A copy of the registered order and other related documents are attached to this notice.

A registered child custody determination is enforceable in Wisconsin as of the date of the registration, in the same
manner as an order issued by a Wisconsin court.

If you wish to contest the validity of this registered out-of-state child custody order, you must request a
hearing within 20 days after the date that this notice was mailed to you. Your request for a hearing must be
in writing and filed with the Clerk of this Court at _________________________________________________

If you do not request this hearing, within that 20 day period, the out-of-state order will automatically be
confirmed in Wisconsin (and may be enforced by Wisconsin courts), and you will not be permitted to
challenge the order in the future with respect to any matter that you could have asserted.

At the hearing, the court will confirm the out-of-state order unless you can prove that:
a.
The issuing court did not have jurisdiction under Wis. Stats. Ch. 822, Subch. II; or
b.
The out-of-state order has been vacated, stayed, or modified by a court having jurisdiction to do so
under subch. II of Wis. Stats. Ch. 822; or
c.
You were entitled to notice of the original order, but did not receive that notice in accordance with the
standards of Wis. Stats. s. 822.08 in the proceedings before the court that issued the order for which
registration is sought.
