If custody, visitation with, and/or removal of minor children from the state of Illinois is an issue, the following
is required prior to the case management conference:
The completion and filing of a pre-mediation questionnaire seven days prior to the case management conference.

Both parties having completed the Children First parent education program as required by local rules.
Completion and filing of financial affidavit seven days prior to the case management conference(forms are
available from the Circuit Clerk).
THIS SETTING IS SEPARATE FROM THE SUMMONS WHICH HAS BEEN SERVED.
FOLLOW INSTRUCTIONS ON THE SUMMONS REGARDING YOUR ANSWER OR APPEARANCE.
IF YOU DO NOT FILE AN ANSWER WITHIN 30 DAYS AFTER SERVICE OF THE SUMMONS, YOU MAY BE
DEFAULTED AND A JUDGEMENT ENTERED AGAINST YOU. IF THAT OCCURS, THE CASE MANAGEMENT
CONFERENCE WILL NOT TAKE PLACE AND THE DATE AND TIME FOR THE CASE MANAGEMENT
CONFERENCE WILL BE VACATED.
