It must be signed by the Parent/Guardian or person acting in loco parentis.

Risk Statement

Sailing is by its nature an unpredictable sport and therefore inherently involves an element of risk.

By allowing my athlete to participate in this event I, the parent/guardian of the athlete mentioned above agree and

acknowledge that:

a) I am aware of the inherent element of risk involved in the sport and accept responsibility for exposing my athlete to

such inherent risks;

b) I have satisfied myself that my athlete has the necessary skill and knowledge to take part in this event and to deal

with conditions that may arise in the course of a race

c) I will not allow my athlete to participate in this event whilst under the undue influence of alcohol, drugs or whilst

otherwise unfit to participate

d) I am responsible for ensuring that the boat is in good order, equipped and insured (including third party insurance

as stated in the NOR of the event) to take part in this event.

e) I am responsible for my athlete’s property whether afloat or ashore;

f) I accept responsibility for any injury, damage or loss to the extent caused by my own actions or omissions or actions

or omissions of my athlete;

g) the provision of a race management team, patrol boats and other officials and volunteers does not relieve me of my

parental/guardianship responsibilities or my athlete of his/her responsibilities;

h) the provision of patrol boat cover is limited to such assistance, particularly in extreme weather conditions, as can be

practically provided in the circumstances;

i) I consent to my athlete participating in drug testing procedures and if asked providing a urine sample under

observation for analysis at an accredited laboratory

j) My athlete is bound by the Notice of Race , Sailing Instructions and Racing Rules of Sailing (save as amended by

the Notice of Race or Sailing Instructions)

Parent/Guardian/Loco Parentis Declaration:

I confirm I am the legal guardian of this competitor or have been asked by the legal guardian to act in loco parentis. I

accept the Risk Statement above and Disclaimer of Liability below, which excludes said dependent’s right to claim

compensation in certain circumstances. I confirm that said dependent is competent to take part. I consent to said

dependent participating in drug testing procedures and, if asked, providing a urine sample under observation for

analysis at an accredited laboratory.

During the event (tick one):

 I will be responsible for my dependent throughout the event, and during the time my dependent is afloat I will be

available at the event venue (“venue” includes either on land and or water) .

 The person named below, who has agreed to act in loco parentis will be responsible for said dependent throughout

the event. During the time said dependent is afloat he/she will be available at the event venue*.

Disclaimer of Liability:

Athletes are entirely responsible for their own safety, whether afloat or ashore, and nothing reduces this responsibility.

Nothing done by the organizers can reduce the responsibility of the owners and/or athletes, nor will it make the

organizers responsible for any loss, damage, death or personal injury, however it may have occurred, as a result of

the boat taking part in the racing. “The organizers” encompasses everyone helping to run the event. The provision of

rescue boats does not relieve owners and athletes of their responsibilities.
