Advance Directive Forms

The following forms include a Health Care Directive and a Durable Power of Attorney. These are

considered advance directives. It is helpful to talk with those you are close to when making decisions

about advance directives. It may also be helpful to seek advice from an attorney. Please take some time

to review the information in this booklet about these forms and consider whether or not you want to

complete either or both of the forms.

If you do complete the form, it is important to talk to your health care provider and ensure that a copy is

provided for your medical chart. If you wish to have a copy included in your UW Medical Center chart,

bring the form in to your provider at your next clinic appointment, or mail the form to:
