Subway Thursdays

Subway sandwiches will be available on Thursdays this semester, by pre-order, starting February 3, 2016,

going through Thursday, May 19, 2016. The lunch includes a 6” sandwich (veggie, turkey, ham, or Italian) and

a choice between chips or a cookie. The cost is $4.75 and must be ordered and prepaid by Friday, January

29, 2016. Please use the order form below to order your choices. All profits are directed to TSMV’s Athletic

Program. GO TEMPEST!!!

Please note that refunds for occasional absences are not available.

This order form covers 13 Thursdays: 2/4/16 through 5/19/16, excluding Winter Break, Holy Thursday and

Easter Break. SUBWAY SANDWICHES MUST BE PRE-ORDERED AND PRE-PAID. Any leftover

sandwiches will be sold on a first-come, first-paid basis, after all pre-orders have been filled. Servers can’t give

sandwiches to students who haven’t ordered and paid, nor can they run a tab.

This form is due at the school by Friday, January 29, 2016. Checks may be made out to Trinity School

at Meadow View. Please indicate “Subway” on the memo line. We are not able to accept a credit card for

payment. Please mail the form and a check to school:
