CLAYTON PARK JUNIOR HIGH SCHOOL

SUBWAY LUNCH ORDER FORM

The next session of the Subway Lunch program will be 6 weeks long and will begin on

November 5

and end on December 17th. (There is a PD day Nov. 26

; therefore

that week will be skipped) Subway Day will take place each Thursday during this 6

week period.

If you would like to take part in this program, please fill out the order form below and

return it with payment in full to the school office by Monday, October 26th, 2015. Please

be advised that late orders cannot be accepted as the orders must be delivered to

Subway in time for them to process. On Subway Day, orders will be individually

packaged, labelled with student's name, and distributed in time for lunch. Please make

arrangements if your child will not be in school on a Subway Day as unclaimed

sandwiches will not be saved.

Order form instructions:

1. Fill in child's name, grade, and class.

2. Place an “X” or a “check mark” for the choices you want to order.

3. Tally your weekly amount due (based on listed prices for what you ordered) and

multiply by 6 for the total amount due. Orders must be for the full 6 week period.

4. Only one order per form can be accepted. You must submit a separate order form for

each child you have at the school.

5. Payment in full must be turned in with each order form. Payment must be for the

exact amount due as we are unable to provide change for overpayments and we cannot

accept orders that are short. Checks are to be made payable to: Clayton Park Junior

High or CPJH.
