• The search fee is $5.00 for each year/period requested or $6.00 per each year/period if certified copies are needed.

USE THE FOLLOWING CHECK LIST TO MAKE SURE YOU INCLUDE EVERYTHING WITH THIS REQUEST

For the person whose returns are requested include:
- Copy of identification containing photograph and signature of the person whose returns are requested, (e.g. driver's license, state
identification card, passport, or military identification.)
- Copy of identification containing social security number and signature of the person whose returns are requested, (e.g. U.S.
Government issued social security card, Medicare Insurance card, or Internal Revenue Services (IRS) authorization for an individual
taxpayer identification number (ITIN).
b. If the name on the identification provided is different than the name listed on the tax returns, submit documentation of proof of the
name change (e.g., marriage certificate, divorce decree, etc.).
If you are not the taxpayer, you must also submit a copy of a properly executed Power of Attorney, Form A-222, as authorization to

receive this material. You may also use a tax authorization form or IRS Power of Attorney but those documents need to have the
taxpayer's signature notarized (including notary stamp/seal).
If the taxpayer is deceased, and there is an estate, include a certified copy of the domiciliary letter. If there is no estate, include a

certified copy of the death certificate and a letter indicating the reason you need these documents.
If you are an officer requesting a business tax return and did not sign the actual business return, submit proof that you are an officer
