Instructions for Progress Monitoring form
Students that are identified as LEP, MLEP, Migrant or Homeless are at greater risk of
struggling academically than their peers. By mandate, schools are required to monitor their
progress. District 91 will review these student’s progress 3 times per year – mid-term each
trimester.
Review the student’s Quick lookup screen from PowerSchool.
Review the student’s Performance Indicators in Mileposts.
The team (to be determined by the school principal) convenes and reviews each identified
student’s grades, attendance, and assessment results to determine if the student is struggling
and to implement interventions.
Document the student progress and interventions by completing the Progress monitoring
form.
Send the hardcopy progress monitoring form to the district Student Achievement and School
Improvement (SASI) office. The SASI office will scan and maintain evidence of compliance.
Once the form is scanned it will be returned to the school for use in the subsequent
