We are unable to continue processing your 2013-2014 financial aid application because your Free Application for
Federal Student Aid (FAFSA) contains incomplete or conflicting student asset information. As a result, our office
requires that you complete, sign and submit this form to the Financial Aid Office as soon as possible in order to
continue processing your application.
Please be advised that your asset information must be correct as of the date you signed your original 2013-2014
FAFSA. This date can be found on your Student Aid Report. When reporting your assets you should provide the exact
dollar amount. If you give a range, we will use the higher amount. If any item does not apply to you please write “0” in
the space provided.
