INFORMATION FOR BACKFLOW TESTING IN THE TOWN OF ADDISON
The Town of Addison requires all backflow prevention assembly testers to register. Test results from
testers not registered will not be accepted.
To register as a backflow prevention assembly tester with the Town of Addison, a registration form
must be completed and submitted with the following documentation:
TCEQ BPAT License
Current testing gauge calibration report
Certificate/License if you are a licensed plumber
Certificate/License if you are a licensed lawn sprinkler contractor
Confined space Training Certificate (If applicable)
Photo identification
Registration forms can be completed in person at the Infrastructure Operations and Services
Department, located at the Town of Addison Service Center 16801 Westgrove Drive, Addison, Texas
75001. You may also print the registration from our website
(
under the
Infrastructure Operations and Services Department section). An annual non-refundable registration
fee of $75.00 (payable by check, money order, cash or credit card) is required. Town of Addison
Backflow Prevention Assembly Test and Maintenance Report must be used and are available for
$25.00 for a pack of 30 test forms in triplicate.
Upon submission and approval of the registration form by a Town Official, the tester will be added to
the approved Backflow Assembly Tester List which is available to all Utility customers unless tester
requests to be left off of the list.
Backflow Prevention Assembly Testers may test and repair assemblies on firelines only if they are
permanently employed by an Approved Fire Sprinkler Contractor. The State of Texas Fire Marshall’s
Office requires that any person performing maintenance on firelines must be employed by an
Approved Fire Sprinkler Contractor.
It is the responsibility of the tester to ensure that all documentation submitted is current. If the
documentation is not up to date, test reports will not be accepted. Tester registration is good for one
calendar year from the date of the registration.
