Changes to Sport /Charter Fishing Vessels

Change of Information Form

CFEC records indicate you own at least one vessel designated in 1999 as being a "sport fishing charter" boat.

Since the 1998 season, CFEC has required additional information regarding your "sport fishing charter" opera-

tion. This season we only need this form completed if you are changing any of the information such as busi-

ness license number, Coast Guard number, your address , primary port of operation, or areas of operation. If

the information is the same, this form is not necessary.

If the information in CFEC records is incorrect and your vessel is NOT used for charter sport fishing, please

cross out "charter" where listed as a vessel activity on the 2000 renewal form.

If you own more than one sport fishing charter vessel you may complete just ONE of these forms IF the infor-

mation is the same for all vessels. Please list the ADF&G numbers of all vessels to which the information below

applies. If you have any questions, please contact the licensing section at (907) 789-6150 before mailing your

renewal.

2. If the USCG documentation or registration numbers of the vessel(s) have NOT been provided to the CFEC previously,

please be sure to write them in on the preprinted renewal form.

3. Please provide the Hull Identification Number (HIN) for the vessel(s) on the renewal form. This number consists of 12

alpha-numeric characters - on vessels built since 1972 it should appear on the lower right, outboard side of the transom

above the water line.
